An Additional Diagnosis - Torin's Medical Journey

An Additional Diagnosis with Updated Treatment Plan In September, Torin was referred to a Pediatric Endochrinologist in order to receive endochrinology clearance prior to his surgery in Chicago. After discussing the ongoing management and the stability of his Hypothyroidism diagnosis (a very commonly-seen hormonal disorder in individuals with Down syndrome), we debated whether to schedule an ACTH (Adrenocorticotropic Hormone) Stimulation Test at the Pediatric Daycare Unit at Victoria General Hospital to check Torin's adrenal response to stress. About a week after the stim test, we received a call from Torin's Endochrinologist. Much to their surprise, Torin's cortisol levels barely changed AT ALL following ACTH administration. He was diagnosed with Adrenal Insufficiency, also knows as Cortisol Dependency. So where are we at now? It's a HUGE relief that this condition was caught prior to Torin's liver surgery. We quickly begin daily oral Predinisone treatments to help manage his cortisol levels, and we have a modified emergency plan in place that involves immediate intramuscular injection of steroid hormone in the case of a medical illness or emergency. Such an emergency could be a severe head bump, a fever with vomiting, a broken bone or a car accident. The injectable steroid will help Torin's body cope with the stress associated with a traumatic event, keeping his blood pressure and blood sugar levels stable. It could absolutely save his life in the case of a severe trauma. We also have a modified plan in place for the surgery to fix his Portosystemic Liver Shunt in Chicago at Lurie Children's Hospital this November. He will receive an increased "stress dose" or oral Prednisone (steroid) in advance of his surgery and prior to invasive tests or general anesthetics. Throughout his procedures and surgeries, his cortisol levels will be closely managed and he will receive IV doses of steroids while unconscious. We are so relieved to be armed with this new diagnosis and with better knowledge of how to help keep Torin safe and healthy: both during his day-to-day activities, and throughout his upcoming international medical treatments. As always, we are extremely grateful to have access to such exemplary healthcare in our city, as well as amazing medical specialists to help manage Torin's health and wellness.

We are pleased to announce a very special Storytime+PLAY at The Meet Up at Momease, happening Friday, Sept 29th from 1-3 PM. In honour of the National Day for Truth and Reconciliation, we will be welcoming Special Guest Sky Bellefleur from the Victoria Native Friendship Centre for an afternoon of stories and puppetry. The storytelling will be age-appropriate for little ones from 2-5 years, and will focus on amplifying Indigenous voices and stories.Sky works at the Victoria Native Friendship Centre with the Health Team, the Bruce Parisian Library, the Career, Employment and Education Department, and also with the Elders Programs and the Indigenous Language and Culture Team. Sky enjoys reading stories for the Bruce Parisian Library in their Storytime Corner, and is featured on their Facebook page. Sky has lived in Canada for more than 40 years, and has lived on Vancouver Island for 5 years. Here’s what you need to know! How To Choose The Best Stroller For Your Lifestyle If you’re finding yourself spending hours (and hours, and hours) researching strollers online, then it’s time to step back. New moms keep a seemingly endless to-do list running in the back of their minds at all times, and safety is usually at the top of this list. Yes, it would be nice to have the perfect colours for your nursery or a collection of adorable (and locally-sourced!) bibs all ready to go. But safety, of course, is more than something ‘nice to have.’ It’s a necessity.  When thinking about safety for new babies, many people automatically think about strollers and bedding, two of your most important safety purchases. And of course, all the car seats and boosters, strollers, bassinets and bedding in our Victoria boutique are all from highly-vetted, highly regarded brands.  But in this post, we’re going to explore some less-discussed but equally important safety items for your new baby. These items will help you have peace of mind without breaking the bank.  Safety Items For Your New Baby: Suncare We all know that too much sun is bad for babies. But you might not all know just how bad. According to the Skin Cancer Foundation, your baby’s skin is particularly vulnerable to sun damage, and there is “strong evidence that sun exposure and sunburns during childhood multiply the risk of one day developing skin cancer, including melanoma.” Thankfully, keeping the sun off your baby’s skin has never looked so cute!   For example, we love these Jan & Jul Xplorer Sunglasses, which have UV 400 lenses that offer 100% UVA and UVB protection. Photograph By TENILLE VILLEBRUN Many parents are struggling to work from home while serving as homeschool teachers to their kids during the COVID-19 pandemic, but disability advocates and parents of children with special needs say the challenges are even greater for that vulnerable population. Lyra McLean’s son Torin has Down syndrome, and she says not having a predictable classroom schedule or one-on-one support from an educational assistant has been hard on the energetic five-year-old. “He really thrives when he has predictability in his day,” said the Saanich mom. “It’s created a lot more anxiety and stress and it’s created behavioural challenges for him.” Torin’s regular in-person visits with occupational and speech therapists through Queen Alexandra Centre for Children’s Health have switched to video chats, which have proven challenging, she said. “His interest and his ability to be focused on a computer is so limited,” McLean said. Ellen Tarshis, executive director of Community Living Victoria, said family-support workers have been hearing from parents of children with disabilities about the difficulties of online learning. For example, a child is so fixated on the screen, they have a meltdown any time the computer is taken away. In another scenario, a youth with autism struggles with change and is confused as to why his mom is helping him with his school work. “He consistently refuses to do schooling and says: ‘You’re not my teacher,’ ” Tarshis said. The non-profit agency has also heard from parents with mental-health challenges who are facing the added pressure of homeschooling, Tarshis said. “Parents are working from home and trying to homeschool a child with a disability — they’re just not managing both, so often the school work does not get completed,” she said. “Overall, with our families, the children with very high needs who require one-to-one support are really struggling with the online schooling.” In some cases, parents might need to put aside the school curriculum and find everyday learning opportunities for their children, she said. That could include building something, planting a garden or going for a hike and incorporating math or science. Not all children with high needs are having difficulties, however. Tarshis said some can focus better at home without the distraction of classmates. “Some of these children, they struggle socially, they have a tough time making friends or figuring out how to fit in and school can add to that anxiety,” Tarshis said. Family-support workers have also been delivering groceries and helping out with errands, she said, especially for families that are reluctant to go out because their children have higher health risks. McLean has the option of sending Torin back to his preschool at Maria Montessori Academy part-time in June, but since he’s immune-compromised due to asthma, she doesn’t want to take the risk. Torin wouldn’t be able to follow the physical-distancing guidelines at school, McLean said. “As much as it would be nice for him to go back to school I wouldn’t want him to possibly be exposed [to the virus],” she said. McLean has four children between the ages of two and 11, and the family made the decision together not to send her two older sons, Kingston and Lochlan, to school this summer. The family gets in-home help from a nanny while McLean operates the Momease Baby Boutique retail store and her husband works from home at his banking job. McLean has been able to access funding through the Queen Alexandra Centre for Children’s Health that will allow an educational assistant to come to their home to support Torin, who loves to bounce on the backyard trampoline, put on oven mitts to cook at his play kitchen and dance to Baby Shark. “He really does need that stimulation and that one-on-one work.” The Ministry of Education says children who require additional in-class support have been able to remain in school full-time and that will continue into June, when phase 3 of the province’s restart plan will allow kindergarten to Grade 12 students to return to school part-time. The ministry said over the past two months, parents have been able to access the website Keep Learning with free resources that complement the B.C. curriculum and support remote learning for students. “In April, inclusive education specialist Shelley Moore partnered with the ministry on a twice-a-week video series to support children with diverse abilities,” the ministry said in a statement. “Ministry staff have also been meeting weekly with inclusive education organizations (Inclusion B.C., B.C. ED Access and Family Support Institute) to discuss challenges families are facing and develop solutions.” kderosa@timescolonist.com

It can be hard to know what to say, but at least us parents are all in this together! When discussing this current COVID global health crisis, consider working together on a puzzle of the world, consulting a globe if you have one, or reviewing a map of the world online. Reading a special book together (we love "Here We Are: Notes for Living on Planet Earth" by Oliver Jeffers) can also be helpful and thought-provoking, working to open up the lines of communication with our bigger kids and aid discussions and question time.

OUR TRIBE // ♥️ We are so thrilled to share our new Milestone Family Film with you (link in the write up below). 🌿 Filmed on September 1st at the University of Victoria (where Adam & I met 21 years ago at our very first class, on the very first day of university), shooting this video with the talented Tenille at Bebe Films was such a special event for our family. ♥️ It may be cliché to say, but it’s so true that we easily forget so many little things about day to day life: where we were at a particular moment in time; how our little ones smiled while they took their first few steps; the sound of our children’s voices calling out in play. These are the little things that make this film so precious to us - Tenille captured the spirit of our family in a way that we couldn’t have imagined, and we are so pleased to be able to treasure this video over the years to come. 🌿 If you’re looking for a unique way to document the birth of a new baby, a first birthday, first steps, or even just a day-in-the-life of your family, I highly recommend looking into a family film by this amazing local mama!
